The air handler motor has quit working and must be replaced. How do I find the part number?

York Diamond 80, appoximately 15 years old. Problem started 2 years ago, replaced capacitor and fixed previously, this time no-go. Have to replace motor for air handler but need part number. Where/how do I find the number?
